FYI: Szaniawski casualty questionnaire

If you (intrepid 355th FG researcher) have been unable to locate the first page of Edward W. Szaniawski's casualty questionnaire you can find it misfiled as page 21 of MACR 3241 at Fold3.com (footnote.com).
 
The page has a numerical notation in hand "49902-3241" which apparently caused this filing error.
 
I couldn't locate the correct MACR number for Col. Szaniawski's incident.
